45 year old man with focal neurologic deficits, headache and fever for several weeks.
There are multiple irregular, pale necrotic areas amassing in the right basal ganglia region with protrusion into the ipsilateral lateral ventricle. The opposite side shows similar lesions, although less extensive. Whitish chalky foci of calcification are discernible. There is mild dilatation of the lateral ventricles (search for basal meningeal involvement with blockage of CSF flow).
