66 year old man with chronic cough productive of copious greenish, foul-smelling, purulent sputum, especially in the morning after waking up. The sputum is occasionally blood-stained.
Examination of the fingers reveals clubbing.
The lung shows a conglomerate of cystically dilated bronchial structures with thickened walls. They communicate with the main bronchus (unlike true cysts). They contain foul-smelling purulent exudates before sectioning
