55 year old man with chronic hepatitis B for many years. Imaging revealed a large mass and multiple smaller nodules, on a background suggestive of cirrhosis. Serum alpha-fetoprotein was elevated.
The liver is nodular, firm and brownish. Cut surface shows a large, pale, subcapsular mass with a variegated appearance due to necrosis and haemorrhage. There are multiple small, pale satellite nodules in the vicinity.
No portal vein involvement by tumour is seen.
Attached gallbladder is noted.
